Panel appointment with Penawar can be obtained simply by submitting your application via our Online Panel Application.
Special attention is needed when filling up the form mainly;
- The required conditions - we will provide services based on whatever conditions instructed in writing by the company. Therefore it is advisable to write your requirement as detail as possible such as.
- Types of treatment covered by the company.
- Special allocation to certain employees,
- Identification used / honored for treatment ie by using either one or both of Penawar Medical Chit, Borang Pengesahan Rawatan (BPR), IC, company badge etc (Please feel free to attach additional paper to detail your requirement since the space provided within the form is quite limited)
- A copy of Form 49 and Form 24 for our reference. (Where applicable)
We will process your application within 1 week upon receiving a completed submission.
Activation of panel will only valid when you receive a confirmation letter from our office.
